Hi, we are preparing a brief summary of what we have done so far in the Mental Comfort Program and we are planning our next steps. We want to receive your feedback on what we have prepared for you so far in this program.
The survey takes about 10 minutes to complete. We've included direct links to the materials we're asking about in the survey to make it easier for you to recall them. If you have not been introduced to these materials before, you can skip a question and come back to us with feedback later after you have become familiar with the material.
Most of the questions are closed-ended, open-ended questions are optional, but we would appreciate your feedback if you would like to share it with us. Any feedback will help us develop our skills and services. Please note that your answers are fully anonymous.
